According to its website, SurveyMe allows you to create contact surveys with different types of data collection options, including but not limited to checkboxes, radio buttons, textboxes and text areas.

The functionality of SurveyMe allows you to determine which areas of your survey are mandatory. For instance, if you want new unregistered visitors to be able to fill out the survey, you can do so while requiring them to include an email address for future correspondence.

Another handy aspect of SurveyMe is that it contains a simple CAPTCHA to help prevent SPAM.
